If your child is having surgery at Center One:

  3. On the Center One Website you can find directions to the surgery center and other helpful information.
  4. Please bring this completed form with you the day of surgery: History and Physical Form  (needs to be filled out by your pediatrician within 30 days prior to surgery).

  6. A nurse from Center One surgery center will call you between 2PM-4PM the business day prior to surgery to let you know what time to arrive, feeding instructions, and answer any questions that you may have.

  9. Pre-registration for Wolfson must be done 1-2 weeks prior to surgery. You may call 904-202-2280 M-Thur 8AM-4:30PM.
  10. A nurse from Wolfson will call you after 4:30PM the business day prior to surgery to let you know what time to arrive, feeding instructions, and answer any questions that you may have. If you do not receive a call by 5PM please call Wolfson 904-202-8838.
