Scheduling
- Surgery scheduling is done via email. Within 72 hours of your office visit, you will be contacted via the email address you provided on the surgery contact form at checkout.
- The title of the email will be SURGERY – LAST NAME (example: SURGERY – JONES)
- Please search your inbox and spam folder using the email title or the scheduler’s email address.
- If you are unable to locate your email after 72 hours, you can email the scheduler directly.
- If you are scheduling with Dr. Black, please email Tish at TBATEY@FLORIDAPEDIATRICS.COM
- If you are scheduling with Dr. Simonsen, please email Allison at AROCHE@FLORIDAPEDIATRICS.COM
- If your doctor is planning to coordinate a surgery with another doctor, it does take additional time. If you have questions regarding a coordinated surgery for either doctor, please email Allison at AROCHE@FLORIDAPEDIATRICS.COM

If your child is having surgery at Wolfson Children’s Hospital Downtown

Pre-Op Physical (Also called H&P)
Schedule your pre-op physical with your child’s pediatrician within 30 days of surgery. Physicals older than 30 days will not be accepted and will have to be redone. Please remind your pediatrician to fax a copy to both numbers at the top of the page and bring a copy with you on the day of surgery. It is very important, because it provides clearance for anesthesiology. You may not proceed with surgery without the pre-op physical/clearance from the pediatrician/primary care. Refer to the parent responsibilities checklist/surgery information page for additional details about the required pre-op physical.
Post-Op
- If you have medical, post-op, or prescription questions please contact the clinical coordinator at 904-398-5437 extension 314
- If your child’s surgery requires post-op medications, they will be sent to the pharmacy on file a few days before surgery. The post-op medications are to be started after surgery.
- You will be contacted by our office on the day after surgery to check on your child’s post-surgical progress and schedule a post-op appointment. Please contact our office at 904-398-5437 extension 314 with any postoperative medical questions or concerns, or if your child becomes ill prior to surgery.

The answers to most, if not all, questions can be found in the scheduling emails or documents that your scheduler sends. If you have a question that is not answered in your scheduling emails, surgery documents, or below, please contact your scheduler.
What happens after I schedule surgery?
After choosing a date, you will get a confirmation email with next steps. Pre-op information will be sent at this time via Adobe Sign. See What paperwork do I need to complete prior to surgery? below.
What time will my child’s surgery be?
OUR OFFICE DOES NOT PROVIDE SURGERY TIMES. This information is provided by the OR nurses at the facility. Please review your pre-op documentation for more information after scheduling. The facility will give you directions regarding arrival time, surgery start time, as well as instructions on when your child must stop eating/drinking prior to surgery.
- You will receive a call from the facility the day before surgery if your child is scheduled at Center One Surgery Center or Jacksonville Surgery Center. If surgery is on a Monday, they will call on Friday.
- You will receive a call from the facility 2 days before surgery if your child is scheduled at Wolfson. If surgery is on a Monday, they will call on Thursday.
What paperwork do I need to complete prior to surgery?
You need to complete the above mentioned documents that will be emailed to you from Adobe Sign. This is an electronic click to sign platform. You will receive a link to view this document once it is signed, as well as a PDF copy. PLEASE LET YOUR SCHEDULER KNOW IF YOU DO NOT RECEIVE AN EMAIL FROM ADOBE SIGN WITH A COPY OF YOUR DOCUMENTS AFTER SIGNING! IT IS VERY IMPORTANT THAT YOU HAVE THESE DOCUMENTS TO REFER TO! Your Adobe Sign document contains:
- The Surgical Financial Policy Form, which contains fees and information pertaining to surgery as well as surgical financial policies.
- FOR SURGERIES SCHEDULED AT WOLFSON ONLY: A Pre-Anesthesia Questionnaire, which will be reviewed by the anesthesia department.
- A checklist/information sheet about your child’s surgery.
- A blank pre-op physical form for you to take to the pediatrician/primary care provider.
What will my child’s surgery cost?
- Financial information will be included in the Adobe Sign documents you receive after scheduling. You can review this example for information on fees associated with surgery.
- OUR OFFICE DOES NOT PROVIDE INFORMATION REGARDING HOSPITAL/SURGERY CENTER/ANESTHESIA FEES AT ANY TIME. We do not have access to this information. You will need to speak with the facility.
- THE FINANCIAL INFORMATION/SURGERY DEPOSIT ESTIMATE PROVIDED IS FOR OUR OFFICE ONLY. Please review your copy of the Surgical Financial Policy Form, which contains the fees and information pertaining to your child’s surgery as well as surgical financial policies. Please read over this carefully so you understand all fees relating to the surgery deposit,
cancellation, and rescheduling. - The deposit provided on this form is calculated based on your insurance and may be based on your deductible, co-insurance, or co-pay. The deposit/fees provided are estimates only! Your final financial responsibilities will be determined by your insurance company.
What if I am not the child’s biological parent?
A biological or adopted parent must be present for the child’s surgery. If your child is accompanied by or under the care of anyone other than the guardian, proof of legal guardianship must be presented prior to surgery. Surgery can not be scheduled if the court paperwork has not been provided and approved in advance. Your scheduler will guide you on how to obtain a court order from your caseworker to proceed with surgery scheduling.
